Corrosive Gastritis
If you accidentally (or deliberately) swallow a caustic substance such as lye or acid, it does not usually reach the stomach but results in severe inflammation or damage to the oesophagus with difficulties in swallowing, producing obstructive scar tissue. But when the caustic material affects the stomach, the consequences are far more serious than alcohol, creating destruction not only of the mucosal barrier but also the underlying layers.
The mucosal barrier can also be worn away by other milder caustics such as fiery or highly-seasoned foods eaten continually. Gastritis is not caused by healthy food and drinks that are too hot in temperature, since by the time these reach the stomach they have ceased to be hot enough to affect the stomach.
Other causes of corrosive gastritis are drugs, and particularly aspirin, especially when taken on an ‘empty stomach.’ Your doctor may prescribe medications for other illnesses, such as antibiotics or those for arthritis, and these can have an inflammatory effect. It is important to clearly understand instructions about prescription or non-prescription medicines and whether they should be taken before, with, or after meals or a small snack.
If the victim has accidentally or deliberately swallowed lye, or if the substance is unknown, this is an emergency when it is essential to see the doctor. In most cases, nothing should be given by mouth, and vomiting should not be induced. If you know that the caustic was an acid, then milk, or egg white may help neutralize it. If the corrosive gastritis is diet related, and fiery foods are to blame, by avoiding these offenders the mucosa usually heals itself within a few days.
Treatment will depend on the severity of the injury to the stomach. If the stomach wall is in danger of perforation or is severely weakened, stomach pumping is not usually performed. The doctor may give antibiotics if an infection is suspected, or treatment may include surgery.
